Evidence and FDA status

The research behind MOTS-c

MOTS-c has a real and growing literature, and it also has clear limits. Both are set out here, with the primary sources linked so you can read them yourself and take them to your provider.

What the science describes

A signal written in your mitochondrial DNA

Almost every peptide is encoded in the DNA in a cell's nucleus. MOTS-c is not. It is a 16 amino acid sequence encoded in mitochondrial DNA, in the 12S rRNA region, which places it in a class called mitochondrial-derived peptides. Your mitochondria are not only producing energy. They are sending signals, and this is one of them.

The published mechanism is well characterized. MOTS-c raises AICAR, which activates AMPK, the enzyme cells use to sense low energy, mostly in skeletal muscle. AMPK then activates PGC-1alpha. That drives the building of new mitochondria, the same adaptation endurance training produces. It is why researchers describe MOTS-c as an endogenous exercise mimetic.

There is a genuinely interesting corollary. Reynolds 2021 showed that exercise raises the MOTS-c your own body makes, and that levels decline with age. Worth being clear that the mimetic label describes a shared pathway, not a replacement: activating AMPK is not the whole of what training does.

Where the evidence stands

Strong in animals, early in humans

The animal work is substantial. Lee 2015 identified the peptide and measured its effects on metabolic homeostasis and insulin resistance in mice on a high fat diet. Reynolds 2021 measured physical capacity in mice across ages. This is where the interest in MOTS-c came from, and it is solid preclinical science.

Human work has begun. CB4211, a modified MOTS-c analog, was taken into a double blind placebo controlled Phase 1 trial and was found safe and well tolerated, followed by a Phase 1b in adults with obesity and raised liver fat.

Two caveats come with it. You should hear them from us. The trial used an analog rather than the peptide compounded here, and Phase 2 development was discontinued. So there is no completed efficacy trial in humans and no long term human safety record yet. That is the honest state of play, and it is why this page describes a molecule and a service rather than a result.

Regulatory status, August 2026. MOTS-c is not an FDA approved medication. There was a notable development on 23 July 2026: the FDA's Pharmacy Compounding Advisory Committee voted to recommend that MOTS-c be added to the section 503A list of bulk substances pharmacies may compound, and did so against the FDA's own briefing document, which had proposed not adding it.

Read that for what it is. A committee recommendation is advisory, it does not bind the FDA, the Secretary of Health and Human Services still has to act, and it is a judgement about compounding suitability rather than about whether MOTS-c works. It does suggest the committee weighed the evidence seriously. The FDA's review also flags immunogenicity, peptide impurities, and how well the raw ingredient is characterized, which is precisely why Las Vegas Mobile IV Therapy dispenses only pharmacy-compounded vials against a named provider's order.

Storing your vial

Store the vial exactly as its label directs, in its labeled packaging, upright and out of direct light. Never the freezer and never the fridge door. Keep it where children and pets cannot reach it. After a power cut or a long spell out of storage, call before dosing rather than guessing, and never use a vial that is cloudy, discolored, has particles, or is past its beyond use date.

Giving yourself a dose

Wash your hands and set out a swab, a new syringe, and your written schedule. Swab the vial stopper, let it air dry, and draw exactly the amount your provider specified. Choose a site your nurse showed you, and rotate sites each dose. Pinch gently, insert at the angle you were taught, inject slowly, then apply light pressure with a clean swab without rubbing, and log the dose and the site. Stop and call if you notice spreading redness, swelling, warmth, hives, or any trouble breathing. If you monitor blood sugar, log those readings next to your doses. MOTS-c acts on glucose metabolism, so that log is the most useful thing you can bring to your provider review. For a medical emergency call 911.

Sharps disposal

Used syringes go straight into the sharps container. Never recap, bend, or reuse a needle, and never put sharps in household trash or recycling. Keep the container upright and closed between uses, stop filling at the fill line, then close the lid permanently and take it to an approved Nevada collection site, or ask your nurse and we will arrange collection.
